Purpose:

  • There are various statutes regulating Safety, Health and Environmental requirements in South Africa that place specific responsibilities not only on the employer but also on the employee
  • The investigation process forms part of every safety management program, however, the purpose of doing investigations is often poorly understood
  • As a result, investigations can be used as finger-pointing, blame-fixing and fault-finding
  • The aim should be to determine the real causes of what actually happened and then implement corrective and preventative actions that will ensure that the incident will not happen again
